Parent/Guardian Consent for Photographic/Video Release Form

My child has my permission to be photographed in the Summer Academic Enrichment Program (SAEP) and all other CSF-DC programming.

 

I further understand that my child’s selection as a College Success Foundation - DC participant may involve coverage by the media. I hereby release any claim I may have surrounding rights to my student’s name, image, voice, or likeness, and I agree that the Foundation, CSF, CSF-DC and Friendship Collegiate Academy and other partner organizations may use my child’s name, image, voice, or likeness in connection with publicity for the College Success Foundation - DC.



College Success Foundation - District of Columbia Student - Parent(s) / Guardian(s) Compact (Agreement)

Congratulations on becoming a candidate for the College Success Foundation–District of Columbia (CSF–DC) Scholars Program! As a CSF–DC Scholar, you will join a talented group of young people uniquely selected to participate in new programming designed to help you complete high school, enter college and graduate with a post-secondary degree. The CSF–DC Scholars Program is a seamless pipeline of academic and developmental supports and services designed to increase your academic readiness and improve your chances for post–secondary success.

 

Participation in the program includes, one-on-one academic advisement and counseling, group leadership development meetings, after-school academic and developmental supports, tutoring, Saturday-based academic enrichment and a six-week summer extended learning experience. The CSF-DC Summer Academic Enrichment Program includes instruction and academic support, field trips, college tours, social activities, and an independent capstone project or performance. Over the course of each year, you will benefit from college preparation assistance, career workshops, and opportunities for Microsoft Certification and a Coding Program. As a CSF–DC Scholar, you will be expected to meet programmatic expectations in five key areas. Expectations include meeting academic performance and citizenship requirements, peer/program communication and interaction, college readiness planning, matriculation guidance, and personal and career development.

 

Parental support and participation in CSF-DC special events and programming are also expected. We encourage and expect parent(s)/guardian(s) to attend CSF-DC Parent University programming at least twice during the academic year, and we invite parent(s)/guardian(s) to visit the school(s) to attend parent/teacher conferences, CSF-DC advisement meetings, and other school based-events as appropriate. These activities will help our parent(s)/guardian(s) stay informed about our students’ academic and college-readiness progression.

 

The College Success Foundation-District of Columbia and our partners will help support you to reach your goal of successfully completing high school and obtaining a college degree. We will support you with timely information on college planning, help you obtain resources to select a college/university, help you network and access resources at your school, and support you through the admissions and financial aid processes for college. Your continued participation with us will ensure your readiness for college and career success. As you and your parent(s)/guardian(s) review the programmatic expectations (attached), you will note that your initials and signature, as well as those for your parent(s)/guardian(s), are required to formally accept our offer of support. Please review the programmatic expectations carefully, and initial your agreement where indicated. Following your agreement to participate, we pledge to support you, and ensure that you are academically, developmentally, and socially ready to achieve your college dreams.

Please review and discuss the programmatic expectations carefully, and initial your collective agreement where indicated. Following your agreement to participate, we pledge to support you, and ensure that you are academically, developmentally, and socially ready to achieve your college dreams.

The Required Key Programmatic Expectations and Agreements: 

 

Academic Performance

                                                                                                                                                                                        I understand and agree that I will earnestly engage and pass all courses as indicated by my middle school and high school curriculum.

 

 I understand and agree to attend required one-on-one academic advisement meetings, participate in group leadership development meetings, participate in academic tutoring and/or support as appropriate, and communicate my progress to both my parent(s)/guardian(s) and CSF-DC advisor.

 

 I understand and agree to meet with my CSF-DC advisor for appropriate academic and developmental supports. This includes recommendations for tutoring, afterschool college readiness activities, one-on- one advisement meetings, monthly group leadership development meetings, and participation in The CSF-DC Level Up, and Summer Academic Enrichment Program.

 

I understand and agree to provide course schedules, interim progress reports, and final grades/report cards to CSF–DC staff and meet with my advisor to discuss my progress and plan next steps. 

 

Citizenship

                                                                                                                                                                                            I understand and agree to be a leader and role model in my school community and I agree to obey the rules/regulations and policies that foster a safe environment in my school and community. 

 

I understand and agree to adhere to all rules and policies set forth by the College Success Foundation- DC, my CSF–DC advisor and program partners.

 

I understand and agree to demonstrate positive behaviors in all CSF–DC programs and services including the Summer Academic Enrichment Program (SAEP), Level Up, and other CSF-DC activities, workshops     and     special     events.


College Planning

                                                                                                                                                                                            I understand and agree to attend mandatory meetings facilitated by my CSF–DC advisor or CSF-DC staff.

 

I agree to participate in all CSF-DC workshops and special events designed to support my college readiness and college admission.

 

I understand and agree to complete all documents and applications for college admission and financial aid on time and as directed by my CSF-DC advisor. This includes the application for FAFSA, The DC ONEAPP/DCTAG, and other scholarship applications deemed appropriate.

 

I understand and agree to apply for admission to at least five (5) colleges/universities as agreed upon with my parent(s)/guardian(s) and my CSF–DC advisor.

 

I understand and agree to prepare for, and take, the SAT/ACT exam during my junior year and to take the exam as often as necessary to improve my chances for college admission.

 

Communication and Interaction with the College Success Foundation-DC

I understand and agree to attend all CSF–DC student special events and activities to include but not limited to The CSF-DC Scholars Program Induction Ceremony, Summer Academic Enrichment Program, the CSF-DC Level Up and Parent University as appropriate.

 

I understand and agree to stay in constant communication with my CSF–DC advisor and provide updates on any changes in my contact information, (name, address, email address, phone number etc.), and academic performance.

 

I understand and agree to submit all documents requested by CSF–DC staff in a timely fashion. Documents include travel/activity permission slips, academic progress reports, report cards, transcripts and course schedules, college applications and essays, admission letters (acceptance and denial), financial aid information/awards, and other documents related to proof of residency and income.

 

I understand and agree if I transfer out of my current school to attend a non CSF-DC participating school, all CSF–DC services and my continued participation in the program will be subject to review and approval.

 

Personal, College and Career Development

                                                                                                                                                                                            I understand and agree to register for, and participate in, the annual Marion S. Barry Summer Youth Employment Program (MBSYEP) during each summer academic enrichment program session facilitated by CSF–DC.

 

I understand and agree to participate in high school to college transition meetings with CSF–DC staff during the summer immediately following my graduation from high school to ensure consistent services and monitoring during my college years. Further, upon acceptance into college/university, I will continue to meet with CSF–DC staff by phone, email or in person on a monthly basis to ensure college-level satisfactory academic performance, course scheduling, and financial aid counseling and support.

 

I understand and agree to participate in all CSF–DC college-level programming and special events. These include the CSF-DC Orientation session,  The Annual Scholars Spotlight Celebration, The Annual CSF-DC College Roundtable and other career readiness and summer “on-track” meetings.

 

I understand and agree to submit semester-based college academic transcripts, course schedules, annual financial aid award information, degree audit(s), and signed FERPA waiver to CSF-DC as requested.


Parent(s)/Guardian(s) Agreement;

                                                                                                                                                                               As a condition of your child’s participation in the program, we welcome and expect parent(s)/guardian(s) to become a critical part of the larger CSF-DC family and participate in special events, college readiness activities, and attend our Parent University programming. Special events include, but are not limited to, the CSF-DC Scholars Program Induction Celebration and the CSF-DC new student parent(s)/guardian(s) orientation meeting.

 

As the parent(s)/guardian(s) of a CSF-DC Scholar, I understand and agree to participate in all relevant CSF-DC and partner based programming in support of my child’s aspirations for college and career success.  

 

I understand and agree to attend important parent/teacher conferences and CSF-DC advisement meetings as recommended by the administrative staff of CSF-DC.

                                                                                                                                                                             

I understand and agree to participate in quarterly CSF-DC Parent University programming at least twice  during  each  academic  year of  my  child’s  participation in the  program.  

 

By signing this CSF–DC Student - Parent/Guardian Compact, I/we agree to abide by all CSF–DC rules, regulations, and policies as mandated by the CSF National Office and all local/municipal agencies. 

 

Further, I agree and permit CSF–DC, and all partners associated with the CSF-DC Scholars Program to review, asses and publish data related to my program participation, demographics, and academic performance as necessary to support my college and career success.
